Position Goal: To manage the Quality and Value-Based Care programs in which Hughston Clinic Orthopaedics participates. Focus will be toward ensuring primary total joint arthroplasty (TJA) patients are guided through the entire surgical and recovery experiences to facilitate improved transitions and outcomes.

  • Manages patient surgical case care for any/all episodes under an alternative payment arrangement program such as, TJR for BPCI and coordinates with physicians and HCO clinical staff.
  • Works with physician lead for Quality Patient Care Programs and assists with communication/meetings between physician lead, HCO physicians and staff, and external care partners.
  • Performs pre- and post-surgical telephone calls to assess patient needs and compliance.
  • Conducts comprehensive pre-admission assessments, education and follow-up to assure needed services are in place.
  • Follows the patient while hospitalized; escalates long stay/high cost cases and actively engages in potential disposition changes to assure appropriate discharge and follow-up.
  • Builds and maintains relationships with care partners in order to achieve stated goals.
  • Builds and maintains relationships with program initiators, such as payers, employers, and facilities in order to achieve stated goals.
  • Monitors, analyzes, and communicates with HCO physicians and staff regarding internal program data and external program reports (as provided by program initiators).
  • Utilizes current patient communication workflow processes using any new systems for this purpose.
  • Reviews all program reports, identifies trends and processes to affect desired program changes, and communicates with physicians and staff regarding reports.
  • Participates in development of departmental goals and objectives.
  • Oversees nursing care essential to the assessment, promotion, maintenance, and restoration of patient’s health and wellness.
  • Maintains patient and staff confidentiality.
  • Assists in the selection, training/orientation, and performance appraisals of department personnel.
  • Recommends departmental policies and procedures. Updates and maintains nursing procedures with in-services or other forms of continuing education.
  • Participates in professional development activities and maintains professional affiliations to include attending conferences and workshops.
  • Responsible for implementation of OSHA regulations.
  • Performs related work as required or assigned.






















Experience: Minimum of 2-3 years of nursing experience in a hospital setting or telephonic case management experience required. Experience with discharge planning and outpatient care coordination preferred.
Education: High School Diploma or equivalent required.  BSN required.
Special Qualifications: Must have TN state RN nursing license. Must maintain BLS certification at all times.  Must comply with HIPAA requirements at all times. Motor Vehicle Record (MVR) must be in good standing.
